If your child is also enrolled in an Office of Child Development aftercare program, you’ll need to submit an activity waiver to the OCD in order for your child to be released to Cornucopia classes.

Returning OCD aftercare students need to submit a new form every term (season).

Your waiver must be submitted to the Office of Child Development. Please do not send your waiver to us at Afterschool Cornucopia. Email your completed activity waivers to: ocdmorningandaftercare@ccusd.org

The PDF form can be filled out digitally, then emailed. Alternatively, you can print it out, fill it out by hand, then scan or photograph it and email that.
